Student Enrollment Process Grades 4K-12
The parent or guardian should be prepared to fill out an enrollment form, student immunization record, health, and emergency cards when registering your child/children. Materials to bring with you when you register include:
- A birth certificate for the child/children being registered
- Verification of address; a utility bill, bank statement, or home phone bill with your correct and current address, or rental agreement
- Immunization records for the child being registered
- If you are enrolling your child/children into the Superior School District from another district either from within or out of the state you will need to fill out a release of records form

If you move to a new home within the District and will be attending a different school before/during/or after a school year, you will need to also fill out a new enrollment form to enroll your child/children into the new school.
